カナダのオンタリオで66歳以上のおよそ140万人の住民を対象とした２つの population-based, nested case-control studies (地域住民を対象としたネスティド・ケース・コントロール研究)が行われました。nested case-control studyは、よく調査された集団 cohortで経過観察中に outcome を持った case 全てと control 全てを把握している場合に行なうことができるそうです。
２年間（2002年4月から2004年３月）で抗菌薬による外来治療後30日以内に1,258名が血糖値異常のため入院となり、マクロライド系抗菌薬と比較して（下記文献より）、

・・・ [ガチフロキサシン] gatifloxacin was associated with an increased risk of [低血糖] hypoglycemia (adjusted odds ratio, 4.3; 95 percent confidence interval, 2.9 to 6.3). Levofloxacin was also associated with a slightly increased risk (adjusted odds ratio, 1.5; 95 percent confidence interval, 1.2 to 2.0), ・・・
・・・ gatifloxacin was associated with a considerably increased risk of [高血糖] hyperglycemia (adjusted odds ratio, 16.7; 95 percent confidence interval, 10.4 to 26.8), but no risk was noted with the other antibiotics.
Risks were similar in the two studies regardless of the presence or absence of diabetes. ・・・

研究デザインの集団とは、下気道感染症の治療のために、しばしば広域スペクトラムの抗菌薬を処方されているオンタリオの66歳以上の外来患者です。
データベースとして、Ontario Health Insurance Plan database 健康保険制度データベース が利用されました。

さて、国内では、ガチフロキサシン（商品名：ガチフロ錠）は、2008年9月30日をもって自主的に販売中止となりました。米国では、2006年6月に商業的な理由から同薬の販売が終了したそうです。
カナダの研究報告の公表から２年余りが経っています。米国では３ヶ月後の販売終了です。
